The Charles B. Wang Community Health Center, an award-winning federally qualified community health center, is seeking Pediatrics Nurse Practitioners for weekend-only part-time coverage. We are looking for team-oriented individuals interested in delivering high quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers.
Responsibilities :
+ Involve with direct patient physical assessment, education, and health planning
+ Maintain continuity of care by regular patient follow-up
+ Assist in the coordination of services with other disciplines and patient's insurance plans
+ Advocate for patient in arrangement of medically necessary services
+ Communicate with community's resources to ascertain adequate assistance for patients in need
+ Refer patients to specialty services when necessary
+ Consult with collaborating physician and other members of the health care team to establish plan of care
+ Participate in quality assurance process to assure optimal care delivery
+ Provide comprehensive case management services that include emphasis on health promotion and disease prevention
+ Monitor patient outcome in accordance to the practice protocols
+ Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning
+ Act as a resource person for the nursing staff
+ Perform other duties as assigned
