Salary: $150,000-$175,000/yr based on experience
LaSante Health Center is seeking an ON-SITE Telehealth NP to deliver essential healthcare services to patients through telemedicine platforms. This role involves collaborating with healthcare teams, assessing patient requirements, diagnosing conditions, and devising treatment strategies.
Responsibilities:
· Plans and organizes patient care according to protocols and pathways.
· Collaborates with the healthcare team to develop and implement treatment plans.
· Conducts health assessments and orders necessary tests.
· Prescribes medications and medical equipment as per practice protocols.
· Evaluates treatment effectiveness and modifies plans as needed.
· Promotes professional growth and serves as a clinical resource.
· Assists Clinical Access Center staff when necessary.
· Completes additional tasks as directed by supervisor
Qualifications:
· License: Current registration as Nurse Practitioner in New York State
· Education: Master's degree in nursing
· Certification as an Adult or Family Nurse Practitioner
